Bluegrass Clinic was formed in 2001. The Stanford Clinic became the first of the two clinics with the Liberty Clinic opening in 2009. Bluegrass Clinic is making an impact to the patients of Lincoln, Casey, and many outlying areas in providing comprehensive family medicine with services to over 5,000 patients between the two facilities.
Bluegrass Clinic has grown immensely with 30 employees, 5 mid-level providers, and 3 Medical Doctors on staff, (Dr. Rodney Bates, Dr. James Miller and Dr. Christopher Sims), providing comprehensive primary care services, hospital, and area nursing home coverage. Bluegrass Clinic has achieved all this in a short time through the vision, leadership, and deep faith based approach of its owners. Bluegrass Clinic has embraced the electronic quality and practices of Meaningful Use and is currently working towards stage 2. Bluegrass Clinic has embraced the quality initiatives that have been promoted by Managed Care Incentives and has recently joined with a ground level ACO startup. Bluegrass Clinic is also active with the Kentucky Primary Care Association.
Bluegrass Clinic has embedded a Licensed Clinical Social Worker as well as two DNP’s with an emphasis on Lifestyle Therapeutics and Obesity into their Health Education model. The focus is not only on traditional behavioral health interventions, but by learning to become more involved in behavior modification and education services to assist their medical patients who struggle to meet self-management goals of their individual care plans. Classes currently offered are 8 Weeks to Wellness, Breathe-Free, Depression Recovery, Managing Diabetes and various healthy cooking classes.
Unique to Bluegrass Clinic is a resident chaplain that is dedicated to minister to clinic patient’s spiritual needs. Providing Spiritual counseling, prayer services, bible classes, and home/hospital visitation helps Bluegrass Clinic accomplish its faith-based mission to help patients to “have life and have it more abundantly” (John 10:10).
Bluegrass Clinic feels that attention to the emotional and spiritual needs of our patients can significantly improve the prevention and healing of disease. These processes are leading the way to be able to achieve chronic care management, and to fully work to coordinate their patients in transitional care between hospitals, nursing facilities, and back home on the road of good health.
Casey County Primary Care
Casey County Primary Care, located at 187 Wolford Avenue directly behind Casey County Hospital, features the services of one physician, Dr. Housam Haddad, and two Advanced Practice Nurse Practitioners (ARNPs), Karen Likins and Bentley O’Dell. The facility is open Monday and Tuesday from 9 a.m. to 6 p.m., and on Wednesday, Thursday and Friday from 9 a.m. to 5 p.m. Appointments may be scheduled by calling 606-787-8348.
Casey County Family Practice
Casey County Family Practice is located at 112 Liberty Square Shopping Center between Family Dollar and Subway. The clinic has two physicians and a physician’s assistant on staff, Dr. Imad Haddad, Dr. Jared Wilson and Hilary Shuffett, PA-C. CCFP is open on Monday and Tuesday from 9 a.m. to 6 p.m., and on Wednesday, Thursday and Friday from 9 a.m. to 5 p.m. Appointments may be scheduled by calling 606-787-5044.
Casey County IOP
Casey County Hospital Intensive Outpatient Program (IOP) is located adjacent to Casey County Hospital. It provides behavioral health services to individuals age 65 and above. IOP assists clients with symptoms of emotional or mental health conditions severe enough to significantly affect their daily functioning. As a component of the patient’s continuum of care, IOP can serve individuals who may be too seriously ill for traditional outpatient treatment and whose symptoms are not severe enough to warrant inpatient treatment. In addition to assisting our clients, IOP staff work closely with the client’s other health care providers and family. Staff available to service clients of Casey County IOP include a Gero-Psychiatrist, Dr. Max Kinnaman; a Physician’s Assistant, Janet Kinnaman; a Psychologist, Dr. John Fulton; Licensed Counselor, Registered Nurse, Patient Aid, and Clerical Workers. The IOP is open Monday through Friday from 8 a.m. to 4:30 p.m. For more information, call 606-787-1460.
Casey County Pharmacy
Casey County Pharmacy, located at 430 N. Wallace Wilkinson Blvd. in Liberty, opened in April 2015. There are two pharmacists, Tonya Moses and David Eubank, and three pharmacy technicians on staff to assist residents with their prescription needs. The pharmacy accepts most major third-party insurance plans as well as Medicare and Medicaid. Flu shots and other immunizations are also offered. Casey County Pharmacy is open Monday through Friday from 9 a.m. to 6 p.m., and on Saturday from 9 a.m. to 1 p.m. For more information, call 606-787-0119.
